Why Science? | FDA Real-World Data, Real-World Evidence and Device Decisions

Three students sit around open books and worksheets at a classroom table, reading, writing and discussing the work together.

eduKateSG · Why Science?

Turn routine health records into evidence that can answer a device question

Use FDA’s December 2025 final guidance to distinguish RWD from RWE, test relevance and reliability, control bias, preserve provenance and support device decisions.

Full chapter index · Science Learning Hub

Health care creates a river of information: electronic records, claims, registries, device logs and observations generated during ordinary care. That river is real-world data, but a large river is not automatically a clear answer. Scientific design turns selected data into real-world evidence for a defined question.

FDA’s December 2025 final guidance explains how the agency evaluates whether real-world data are relevant and reliable enough to generate real-world evidence for medical-device regulatory decision-making. It supersedes the August 2017 guidance and expands the agency’s recommendations for regulatory submissions.

Did you know? “Real world” describes where or how data arise; it does not mean the data are complete, unbiased or ready for analysis. A carefully designed registry may answer one device question well, while millions of poorly aligned records may answer it badly.

This distinction is perfect for science learning. Students can practise provenance, variable definition, confounding, missing-data analysis and reproducibility. Those skills connect secondary science and statistics to career pathways in health informatics, epidemiology, engineering and regulation.

CHAPTER 1 OF 17 · Define the question

1. RWD and RWE are different objects

Back to contents

FDA describes real-world data as data relating to patient health status or delivery of health care that are routinely collected from a variety of sources. Real-world evidence is clinical evidence about the usage and potential benefits or risks of a medical product derived from analysis of RWD. Data are the ingredients; evidence is the result of a fit-for-purpose design and analysis.

The detective work in rwd and rwe are different objects is joyful when every clue keeps its address. Here, FDA describes real-world data as data relating to patient health status or delivery of health care that are routinely collected from a variety of sources. Real-world evidence is clinical evidence about the usage and potential benefits or risks of a medical product derived from analysis of RWD. Data are the ingredients; evidence is the result of a fit-for-purpose design and analysis. The guardrail is: A dataset does not become evidence merely because it is large or routinely collected. That is how routine records become an accountable scientific argument.

Try this

Open a provenance notebook for rwd and rwe are different objects. Write separate definitions for the data source, the analytic method and the decision-relevant evidence output. Add who created each field, why it existed and which transformation changed it. Preserve this limit: A dataset does not become evidence merely because it is large or routinely collected.

A strong science note separates observation from interpretation. Write separate definitions for the data source, the analytic method and the decision-relevant evidence output. 3. A regulatory question comes before a data search

Back to contents

The question should specify the device, intended use, population, exposure or treatment, comparator where relevant, outcome, time window and regulatory purpose. This framework reveals whether a source can supply the needed variables and follow-up. Starting with a convenient database can tempt analysts to reshape the question around whatever fields happen to exist.

Back to contents

Potential sources include electronic health records, medical claims, product or disease registries, patient-generated data and data gathered from sources such as digital health technologies. Relevant RWD contain the population, exposure, comparator, outcomes, covariates and follow-up needed for the proposed analysis. They should be sufficiently representative of the intended use and provide enough observations for meaningful inference. Relevance is therefore relational: it is judged against the regulatory question, not as a permanent property of a database.

8. Provenance keeps the evidence route visible

Back to contents

Data provenance records origin, ownership, collection context, time, versions and transformations. When several sources are linked, provenance should show matching rules and what happens when records conflict.
