Why Science? | ICH M18 and Biosimilar Comparative Efficacy Study Utility

Three students sit around open books and worksheets at a classroom table, reading, writing and discussing the work together.

eduKateSG · Why Science?

Ask whether a large comparative efficacy trial can resolve remaining biosimilar uncertainty after modern analytical, functional, pharmacokinetic and immunogenicity evidence is assembled

Read the current ICH M18 concept and 2026 work plan through totality of evidence, residual uncertainty, assay sensitivity, comparative pharmacokinetics, immunogenicity, study utility and transparent development status.

Full chapter index · Science Learning Hub

A biosimilar is evaluated against an authorised reference product using a stepwise totality of evidence. If powerful analytical and functional studies already detect meaningful differences better than a clinical efficacy trial can, the ethical question becomes whether the trial adds decision-changing information.

ICH M18 is developing a framework for determining the utility of comparative efficacy studies in biosimilar programmes. Its concept paper was endorsed on 19 November 2025, and the March 2026 work plan describes continuing guideline development. There is no final M18 guideline yet.

Did you know? The official concept paper says comparative efficacy studies have historically been a default expectation, while growing experience and modern analytical technologies have raised concern that these trials may lack sensitivity to distinguish product differences and may add little information.

For students, M18 is a brilliant lesson in experimental value: never run a study merely because it is large. Ask what uncertainty remains, whether the design can detect a decision-relevant difference, and what participants contribute that other evidence cannot answer.
