Why English? | Reporting TIDieR-Rehab Interventions Through Activities, Progression, Personalisation, Provider Skill and Fidelity

WHY ENGLISH?

Describe rehabilitation as a changing dose of purposeful activity, not a therapy label

Rehabilitation is often adapted from session to session. The same programme name may hide different activities, equipment, challenge levels, cueing, assistance, progression rules and provider decisions. English matters because another therapist can reproduce or evaluate an intervention only when those moving parts are visible.

TIDieR-Rehab was published in BMJ Open in November 2024. The final checklist contains 22 items under 15 sections: seven original TIDieR items, three adapted items and 12 new rehabilitation-specific items, supported by a supplementary manual.

The extension gives special attention to intended population and timing, planned dose, challenge, regression and progression, personalisation, protocol deviations, fidelity and adverse consequences. Complete description supports replication and optimisation; it does not prove that an intervention works or suits every person.

ORIENTATION

What this guide owns

Current fact check

TIDieR-Rehab was published in 2024. Its 22 items sit under 15 sections and comprise seven original, three adapted and 12 new items, with a supplementary manual and examples.

Canonical boundary

This article owns reporting of rehabilitation intervention descriptions with TIDieR-Rehab. CERT owns exercise-programme reporting, and trial-design or clinical guidelines own effectiveness, safety and care decisions.

Worked example

Every chapter returns to a fictional rehabilitation programme helping students return to school routines after a prolonged health-related absence. It is fictional, low-stakes and educational; it does not report a real school, clinical decision or guaranteed outcome.

Did You Know? For TIDieR-Rehab, complete reporting makes appraisal possible but does not perform the appraisal, remove bias or guarantee transfer.

Warm up with eduKate’s guides to explaining scientific evidence and asking better questions in class.

CHAPTER 1 OF 15

1. Give the Intervention a Brief, Useful Name

Back to contents

Why this move matters

Use a name or phrase that identifies the intervention without substituting branding for description.

Did You Know? TIDieR-Rehab retains the original brief-name item.

Describe what a therapist could reproduce

Use a name or phrase that identifies the intervention without substituting branding for description. Name the essential element, provider action, participant action, equipment, setting, challenge, dose and decision rule instead of relying on a therapy label.

Follow one rehabilitation session

Use The report names a graded return-to-school activity programme rather than rehabilitation support. Trace preparation, active practice, repetitions, challenge monitoring, cueing, assistance, progression or regression, fatigue, safety and actual delivery.

Separate essential elements from support

Distinguish the active ingredients expected to affect outcomes from enabling strategies such as cueing, assistance, transport or reminders. Report personalisation for needs separately from adaptation for preferences.

Keep optimisation and harm visible

A concise name does not reveal dose or active ingredients. Reproducible description helps evaluation and transfer, but it does not prove that the intervention is effective, safe or appropriate for every person and setting.

Practical reader check

  • Point to the exact source or method sentence supporting the claim.
  • Name the person, setting, date, input and decision rule that could change its meaning.
  • Label the remaining uncertainty reported, inferred, contested or unknown.

Student, parent and career transfer

Make one evidence card for “Give the Intervention a Brief, Useful Name”: source location, plain-language meaning, limit and next question. A parent or teacher can ask for one everyday explanation and one specialist explanation; both must preserve the same conditions. This habit supports physiotherapy, occupational therapy, speech therapy, rehabilitation medicine, nursing, psychology, education support and implementation research without promising a grade, admission, funding decision or job.

CHAPTER 2 OF 15

2. Explain Rationale, Goals and Essential Elements

Back to contents

Why this move matters

Describe the theory or goal and identify core components expected to affect outcomes.

Did You Know? The adapted rationale item explicitly names essential elements or active ingredients.

Describe what a therapist could reproduce

Describe the theory or goal and identify core components expected to affect outcomes. Name the essential element, provider action, participant action, equipment, setting, challenge, dose and decision rule instead of relying on a therapy label.

Follow one rehabilitation session

Use The programme links graded task practice and energy planning to participation in school routines.

A plausible theory does not prove which component caused change.

Make one evidence card for “Explain Rationale, Goals and Essential Elements”: source location, plain-language meaning, limit and next question.

CHAPTER 3 OF 15

3. Describe Who the Intervention Is Intended For

Back to contents

Why this move matters

Report condition, stage, function, relevant characteristics and why the intended population can participate.

Did You Know? Intended population is a new rehabilitation-specific item.

Describe what a therapist could reproduce

Report condition, stage, function, relevant characteristics and why the intended population can participate. Name the essential element, provider action, participant action, equipment, setting, challenge, dose and decision rule instead of relying on a therapy label.

Follow one rehabilitation session

Use The programme targets students medically ready for graded activity but unable to sustain a full school day.

A broad diagnosis can hide very different functional needs.

Make one evidence card for “Describe Who the Intervention Is Intended For”: source location, plain-language meaning, limit and next question.

CHAPTER 4 OF 15

4. State When Rehabilitation Begins

Back to contents

Why this move matters

Relate commencement to onset, diagnosis, surgery, discharge, recovery stage or other relevant event.

Did You Know? Timing is a new TIDieR-Rehab section.

Describe what a therapist could reproduce

Relate commencement to onset, diagnosis, surgery, discharge, recovery stage or other relevant event. Name the essential element, provider action, participant action, equipment, setting, challenge, dose and decision rule instead of relying on a therapy label.

Follow one rehabilitation session

Use The programme starts after discharge and a documented readiness assessment.

Calendar time may not match biological or functional readiness.

Make one evidence card for “State When Rehabilitation Begins”: source location, plain-language meaning, limit and next question.

CHAPTER 5 OF 15

5. Make Materials and Equipment Accessible

Back to contents

Why this move matters

List physical and informational materials, specifications, training resources and where they can be obtained.

Did You Know? Materials should be findable, not merely mentioned.

Describe what a therapist could reproduce

List physical and informational materials, specifications, training resources and where they can be obtained. Name the essential element, provider action, participant action, equipment, setting, challenge, dose and decision rule instead of relying on a therapy label.

Follow one rehabilitation session

Use A versioned activity diary, timer and task cards are linked in an appendix.

A generic equipment name may hide resistance, size or configuration.

Make one evidence card for “Make Materials and Equipment Accessible”: source location, plain-language meaning, limit and next question.

CHAPTER 6 OF 15

6. Describe Procedures, Activities and Support

Back to contents

Why this move matters

Write the order, duration and purpose of exercises, tasks, education and enabling activities.

Did You Know? TIDieR-Rehab asks for procedures and enabling or support activities.

Describe what a therapist could reproduce

Write the order, duration and purpose of exercises, tasks, education and enabling activities. Name the essential element, provider action, participant action, equipment, setting, challenge, dose and decision rule instead of relying on a therapy label.

Follow one rehabilitation session

Use Sessions combine route practice, bag organisation, paced reading and recovery planning in a stated sequence.

A session total does not show time spent on essential activities.

Make one evidence card for “Describe Procedures, Activities and Support”: source location, plain-language meaning, limit and next question.

CHAPTER 7 OF 15

7. Report Provider Expertise and Training

Back to contents

Why this move matters

For each provider category, state professional background, rehabilitation expertise, training, supervision and role boundaries.

Did You Know? Provider skill can influence both tailoring and fidelity.

Describe what a therapist could reproduce

For each provider category, state professional background, rehabilitation expertise, training, supervision and role boundaries. Name the essential element, provider action, participant action, equipment, setting, challenge, dose and decision rule instead of relying on a therapy label.

Follow one rehabilitation session

Use An occupational therapist leads task practice while school staff receive a defined support briefing.

Credentials alone do not establish competence in a specific protocol.

Make one evidence card for “Report Provider Expertise and Training”: source location, plain-language meaning, limit and next question.

CHAPTER 8 OF 15

8. Describe Mode, Environment and Infrastructure

Back to contents

Why this move matters

Report individual or group delivery, in-person or remote elements, physical and social setting and required infrastructure.

Did You Know? The adapted environment item includes physical, social, cultural, economic, political and systemic context.

Describe what a therapist could reproduce

Report individual or group delivery, in-person or remote elements, physical and social setting and required infrastructure. Name the essential element, provider action, participant action, equipment, setting, challenge, dose and decision rule instead of relying on a therapy label.

Follow one rehabilitation session

Use Clinic sessions transition to school practice using the same documented task criteria.

The environment can change challenge even when the activity name stays the same.

Make one evidence card for “Describe Mode, Environment and Infrastructure”: source location, plain-language meaning, limit and next question.

CHAPTER 9 OF 15

9. Specify Session Duration and Essential-Element Amount

Back to contents

Why this move matters

Separate total session time from the time or repetitions spent actively performing core components.

Did You Know? TIDieR-Rehab divides amount into several explicit dosage items.

Describe what a therapist could reproduce

Separate total session time from the time or repetitions spent actively performing core components. Name the essential element, provider action, participant action, equipment, setting, challenge, dose and decision rule instead of relying on a therapy label.

Follow one rehabilitation session

Use A 60-minute session contains 25 minutes of graded task practice and 10 minutes of recovery planning.

Session duration alone can exaggerate the dose of active rehabilitation.

Make one evidence card for “Specify Session Duration and Essential-Element Amount”: source location, plain-language meaning, limit and next question.

CHAPTER 10 OF 15

10. Report Frequency and Overall Intervention Length

Back to contents

Why this move matters

State planned sessions per period, home practice, programme duration, breaks and completion rules.

Did You Know? Dose includes frequency and total intervention length as well as session amount.

Describe what a therapist could reproduce

State planned sessions per period, home practice, programme duration, breaks and completion rules. Name the essential element, provider action, participant action, equipment, setting, challenge, dose and decision rule instead of relying on a therapy label.

Follow one rehabilitation session

Use Two supervised sessions weekly are planned for six weeks with three short home practices.

Scheduled frequency may differ from completed frequency.

Make one evidence card for “Report Frequency and Overall Intervention Length”: source location, plain-language meaning, limit and next question.

CHAPTER 11 OF 15

11. Define How Challenge Is Set and Monitored

Back to contents

Why this move matters

Describe subjective and objective indicators of difficulty, effort, intensity or cognitive load and who interprets them.

Did You Know? Challenge is a distinct TIDieR-Rehab section.

Describe what a therapist could reproduce

Describe subjective and objective indicators of difficulty, effort, intensity or cognitive load and who interprets them. Name the essential element, provider action, participant action, equipment, setting, challenge, dose and decision rule instead of relying on a therapy label.

Follow one rehabilitation session

Use Task duration and perceived exertion are monitored alongside symptom recovery.

A harder task is not automatically a better therapeutic dose.

Make one evidence card for “Define How Challenge Is Set and Monitored”: source location, plain-language meaning, limit and next question.

CHAPTER 12 OF 15

12. Write Regression and Progression Rules

Back to contents

Why this move matters

State which dose parameters can increase or decrease, the thresholds, decision-maker and timing.

Did You Know? Progression and regression should include when and how.

Describe what a therapist could reproduce

State which dose parameters can increase or decrease, the thresholds, decision-maker and timing. Name the essential element, provider action, participant action, equipment, setting, challenge, dose and decision rule instead of relying on a therapy label.

Follow one rehabilitation session

Use School-route distance increases after two successful sessions and regresses after a prespecified recovery delay.

Unwritten clinical judgement can make the intervention impossible to reproduce.

Make one evidence card for “Write Regression and Progression Rules”: source location, plain-language meaning, limit and next question.

CHAPTER 13 OF 15

13. Report Personalisation for Needs and Preferences

Back to contents

Why this move matters

Separate supplementary strategies needed for participation from adaptations based on personal preferences.

Did You Know? TIDieR-Rehab divides needs and preferences into separate items.

Describe what a therapist could reproduce

Separate supplementary strategies needed for participation from adaptations based on personal preferences. Name the essential element, provider action, participant action, equipment, setting, challenge, dose and decision rule instead of relying on a therapy label.

Follow one rehabilitation session

Use Noise-reduction support enables participation, while task order reflects the student’s preference.

Personalisation can change the intervention if essential elements are removed.

Make one evidence card for “Report Personalisation for Needs and Preferences”: source location, plain-language meaning, limit and next question.

CHAPTER 14 OF 15

14. Record Protocol Deviations and Actual Fidelity

Back to contents

Why this move matters

Describe what changed, why and when, plus planned monitoring, who assessed it and how much was delivered as intended.

Did You Know? The checklist separates fidelity plan from actual fidelity.

Describe what a therapist could reproduce

Describe what changed, why and when, plus planned monitoring, who assessed it and how much was delivered as intended. Name the essential element, provider action, participant action, equipment, setting, challenge, dose and decision rule instead of relying on a therapy label.

Follow one rehabilitation session

Use A school closure moves practice online; the deviation and completed dose are reported separately.

Attendance does not prove correct delivery of essential elements.

Make one evidence card for “Record Protocol Deviations and Actual Fidelity”: source location, plain-language meaning, limit and next question.

CHAPTER 15 OF 15

15. Monitor and Report Adverse Consequences

Back to contents

Why this move matters

State how physical, mental, social or spiritual harms were monitored and report number, seriousness and relatedness.

Did You Know? TIDieR-Rehab includes planned and actual adverse-consequence reporting.

Describe what a therapist could reproduce

State how physical, mental, social or spiritual harms were monitored and report number, seriousness and relatedness. Name the essential element, provider action, participant action, equipment, setting, challenge, dose and decision rule instead of relying on a therapy label.

Follow one rehabilitation session

Use The report records symptom flares, anxiety and social burden rather than counting only injuries.

Absence of reported harm may reflect weak monitoring.

Make one evidence card for “Monitor and Report Adverse Consequences”: source location, plain-language meaning, limit and next question.

PRACTICE LAB

Build a rehabilitation intervention decision trail

Student route

  1. Choose one original report inside this guide’s scope.
  2. Copy its objective and underline actor, action, context, evidence and intended decision.
  3. Draw the specific reasoning chain used in this article.
  4. Mark each step reported, inferred, contested or unknown.
  5. Rewrite the conclusion so it preserves the narrowest important limitation.

Parent or teacher route

Ask the learner to explain TIDieR-Rehab’s central reporting method twice: once to a classmate and once to a method specialist. Praise a precise correction and a well-stated unknown; both show disciplined attention to evidence.

Early-career route

Create a one-page TIDieR-Rehab audit with source link, date checked, scope, method logic, decisive wording, contrary evidence, limit and next action. A colleague should be able to continue without guessing.

Decision rule

If a shorter explanation removes the condition that made the original claim true, it is not clearer yet. Restore the condition.

FREQUENTLY ASKED QUESTIONS

Questions students and parents often ask

What is TIDieR-Rehab?

A rehabilitation-specific extension of the Template for Intervention Description and Replication.

How many items does it contain?

Twenty-two items under 15 sections.

What is an essential element?

A core intervention component expected to be linked to the outcome of interest.

Why separate session duration from active amount?

A long appointment may contain much less time or repetition of the essential activity.

What is the difference between regression and progression?

They are planned decreases or increases in dose or challenge based on stated rules.

Does complete reporting prove effectiveness?

No. It supports replication and evaluation but does not replace outcome evidence or clinical judgement.
