WHY ENGLISH?
make inclusion, culture, language and equity visible throughout musculoskeletal research from question and recruitment to measurement, interpretation and return
Musculoskeletal research influences how pain, mobility, rehabilitation and participation are understood, yet culturally and linguistically diverse communities can be missing or poorly served by study procedures. English matters because “diverse participants were included” does not explain recruitment, translation, interpreting, health literacy, cultural safety, measurement fit, accessibility, retention, analysis or who benefits from the findings.
DICE stands for diversity, inclusion, culture and equity. EQUATOR records the DICE Checklist project as registered on 14 June 2026 and under development for musculoskeletal research. Its official rationale identifies culturally appropriate recruitment, interpreters or translated resources, community engagement, language, health literacy and culturally suitable data collection as issues that should be considered from the beginning rather than added after data collection.
The associated published DICE scholarship argues that more inclusive research can improve data quality, relevance and utility. Careful reporting should not reduce people to demographic categories or treat English proficiency as intelligence. It should reveal where the study adapted, who remained excluded and how culture and structural conditions shaped interpretation.

ORIENTATION
What this guide covers
Current fact check
The DICE checklist is not final. The live EQUATOR record says under-representation can limit the real-world relevance of findings and that a structured checklist may support recruitment, participation, ethical practice and interpretation. The published 2024 DICE article discusses equity, diversity and inclusion across musculoskeletal research practices.
What this guide does not cover
This article focuses on reporting inclusion, language, culture and equity within musculoskeletal research. It does not rank cultures, assume one community is homogeneous, guarantee representativeness, replace ethics or community partnership, or invent final DICE items.
Worked example
Every chapter returns to a fictional multi-clinic study of persistent knee pain and rehabilitation in adults who use several home languages and have different work, mobility and digital-access conditions. It is fictional, low-stakes and educational; it does not report a real school, clinical decision or guaranteed outcome.
Did You Know? For DICE-informed musculoskeletal research reporting, complete reporting makes appraisal possible but does not perform the appraisal, remove bias or guarantee transfer.
Warm up with eduKate’s guides to explaining scientific evidence and asking better questions in class.
CHAPTER 1 OF 17
1. Define the Musculoskeletal Question and Equity Relevance
Why this move matters
State the condition, care pathway, exposure or intervention and why inclusion, culture, language or structural access could change the question or its answer.
Did You Know? Equity is strongest when connected to a concrete mechanism and decision.
Make inclusion visible across the whole study
State the condition, care pathway, exposure or intervention and why inclusion, culture, language or structural access could change the question or its answer. Connect the population affected, recruitment channels, language access, cultural safety, measurement choices, analysis, interpretation and dissemination instead of limiting diversity to one demographic table.
Trace one participant pathway
Worked example: The study examines knee-pain rehabilitation while recognising that work schedules, transport and language may shape access. Follow invitation, accessible information, language preference, consent, appointment, data collection, follow-up, analysis and return of findings, noting where exclusion could occur.
Keep representation separate from equity
A diverse sample does not by itself prove culturally safe procedures, fair participation or equitable benefit. Inclusion, measurement validity, power, interpretation and implementation require their own evidence.
Respect the development boundary
Adding an equity label does not make every subgroup comparison meaningful. DICE was registered on 14 June 2026 and remains under development. This article uses the official rationale and related published scholarship without claiming a final checklist.
Practical reader check
- Point to the exact source or method sentence supporting the claim.
- Name the person, setting, date, input and decision rule that could change its meaning.
- Label the remaining uncertainty reported, inferred, contested or unknown.
Student, parent and career transfer
Make one evidence card for “Define the Musculoskeletal Question and Equity Relevance”: source location, plain-language meaning, limit and next question. A parent or teacher can ask for one everyday explanation and one specialist explanation; both must preserve the same conditions. This habit supports physiotherapy, occupational therapy, medicine, nursing, rehabilitation, sports science, psychology, public health, community research, interpreting, health communication, biostatistics and research ethics without promising a grade, admission, funding decision or job.
CHAPTER 2 OF 17
2. Describe the Population Affected, Not Only the Sample Recruited
Why this move matters
Use current, respectful categories and explain geography, community context, languages, disability, socioeconomic conditions and who bears the problem.
Did You Know? The people who need the evidence may differ from those easiest to enrol.
Make inclusion visible across the whole study
Use current, respectful categories and explain geography, community context, languages, disability, socioeconomic conditions and who bears the problem. Connect the population affected, recruitment channels, language access, cultural safety, measurement choices, analysis, interpretation and dissemination instead of limiting diversity to one demographic table.
Trace one participant pathway
Worked example: The report distinguishes the clinic sample from the wider communities eligible for rehabilitation.

CHAPTER 3 OF 17
3. Report Community and Consumer Involvement
Why this move matters
Explain who advised, when they joined, selection, authority, compensation, disagreements and what changed in design or interpretation.
Did You Know? A visible change trail shows whether involvement affected the study.
Make inclusion visible across the whole study
Explain who advised, when they joined, selection, authority, compensation, disagreements and what changed in design or interpretation. Connect the population affected, recruitment channels, language access, cultural safety, measurement choices, analysis, interpretation and dissemination instead of limiting diversity to one demographic table.

CHAPTER 4 OF 17
4. Set Inclusion Goals and Monitor Them Prospectively
Why this move matters
State which groups might be missed, why inclusion matters, recruitment expectations, review triggers and what the team will do if access is unequal.
Did You Know? Prospective monitoring makes inclusion a design responsibility.
Make inclusion visible across the whole study
State which groups might be missed, why inclusion matters, recruitment expectations, review triggers and what the team will do if access is unequal. Connect the population affected, recruitment channels, language access, cultural safety, measurement choices, analysis, interpretation and dissemination instead of limiting diversity to one demographic table.

17. Conclude With a Bounded Equity Claim
Why this move matters
State who participated, where access improved, who remained missing, what results support and what future partnership or validation is needed.
Did You Know? A bounded claim is more respectful and more useful than a universal promise.
Make inclusion visible across the whole study
State who participated, where access improved, who remained missing, what results support and what future partnership or validation is needed. Connect the population affected, recruitment channels, language access, cultural safety, measurement choices, analysis, interpretation and dissemination instead of limiting diversity to one demographic table.
Trace one participant pathway
Worked example: The study reports improved language access in three clinics without claiming national representativeness.
Transparent inclusion reporting cannot repair exclusions after the fact.
Make one evidence card for “Conclude With a Bounded Equity Claim”: source location, plain-language meaning, limit and next question.

Build a DICE-informed musculoskeletal research reporting decision trail
Student route
- Choose one original report inside this guide’s scope.
- Copy its objective and underline actor, action, context, evidence and intended decision.
- Draw the specific reasoning chain used in this article.
- Mark each step reported, inferred, contested or unknown.
- Rewrite the conclusion so it preserves the narrowest important limitation.
Parent or teacher route
Ask the learner to explain how English helps us make inclusion, culture, language and equity visible throughout musculoskeletal research from question and recruitment to measurement, interpretation and return twice: once to a classmate and once to a method specialist. Praise a precise correction and a well-stated unknown; both show disciplined attention to evidence.
Early-career route
Create a one-page DICE-informed musculoskeletal research reporting audit with source link, date checked, scope, method logic, decisive wording, contrary evidence, limit and next action. A colleague should be able to continue without guessing.
Decision rule
If a shorter explanation removes the condition that made the original claim true, it is not clearer yet. Restore the condition.

FREQUENTLY ASKED QUESTIONS
Questions students and parents often ask
Is DICE final?
No. EQUATOR lists the checklist as under development after registration on 14 June 2026.
Does a diverse sample prove equitable research?
No. Recruitment, participation, measurement, power, interpretation and benefit also matter.
Is translation enough for language access?
No. Interpreting, comprehension, preferred language and access throughout follow-up may also matter.
Why report interpreter involvement?
The communication arrangement can affect privacy, comprehension and data collection.
Should researchers compare every demographic group?
No. Comparisons need a clear question, adequate data, suitable measures and uncertainty.
How can English support multilingual research?
Clear English can document choices, limits and translations while respecting that participants may communicate best in other languages.

A cheerful final note
Precise English helps us make inclusion, culture, language and equity visible throughout musculoskeletal research from question and recruitment to measurement, interpretation and return more generous, not less. People can see how their evidence and values entered the work, where disagreement remains and what a responsible next step looks like.
