WHY ENGLISH?
make a care-pathway environmental assessment traceable from the clinical decision to activities, inputs, impacts and responsible comparison
Healthcare environmental assessments can look precise because they produce carbon or resource totals. English matters because a number becomes useful only when readers can see the care pathway, patient or service unit, boundary, activities, input quantities, data years, conversion factors, clinical outcomes and uncertainty behind it.
GREEN CARE means Guidelines foR Evaluating the ENvironmental impacts of CARE pathways in healthcare. EQUATOR records the guideline as registered on 3 November 2026. Its 30-point reporting checklist expands on Sustainable Healthcare Coalition guidance and is arranged to follow the stages of a care-pathway environmental sustainability assessment.
The Sustainable Healthcare Coalition describes a pathway approach as including healthcare-related events performed for, on behalf of or by a patient. The 2023 guidance aims for consistent quantification across existing and emerging pathways, while warning that its modules and environmental metrics are limited and should not be used alone as a complete sustainability verdict.

ORIENTATION
What this guide covers
Current fact check
The official GREEN CARE record says the project responds to divergent terminology, incomplete input reporting and inconsistent application of earlier guidance. Development used a targeted review, user-specific recommendations and a structured consensus process with a 20-member scientific steering committee. The current SHC guidance maps direct and supply-chain activity across a patient pathway and dates the evidence and factors used.
What this guide does not cover
This article focuses on reporting environmental sustainability assessments of healthcare care pathways. It does not claim that one impact category represents all sustainability, decide which clinical pathway is best, replace life-cycle-assessment expertise, or invent unreported GREEN CARE checklist items.
Worked example
Every chapter returns to a fictional comparison of two outpatient follow-up pathways: one mostly in person and one combining remote review with clinically necessary face-to-face visits. It is fictional, low-stakes and educational; it does not report a real school, clinical decision or guaranteed outcome.
Did You Know? For GREEN CARE, complete reporting makes appraisal possible but does not perform the appraisal, remove bias or guarantee transfer.
Warm up with eduKate’s guides to explaining scientific evidence and asking better questions in class.
CHAPTER 1 OF 14
1. State the Decision and Intended User
Why this move matters
Name the clinical, operational or policy decision, intended reader and action the assessment may inform.
Did You Know? The same environmental total can support different decisions only when its purpose is explicit.
Place the item on the pathway map
Name the clinical, operational or policy decision, intended reader and action the assessment may inform. Connect the wording to the patient journey, clinical choice, upstream supply chain and downstream consequence. State whether the item is an activity, material, energy flow, journey, waste stream, outcome or assumption.
Rebuild one calculation trail
Worked example: A service team asks whether remote review can reduce travel without delaying escalation. Follow the functional unit, quantity, data source, year, geography, emission or impact factor, conversion and subtotal. Preserve enough English around every number for another team to understand what it represents.
Keep environmental comparison fair
A smaller carbon estimate does not automatically mean better care. Clinical outcomes, access, patient burden, cost, uncertainty and possible burden shifting across impact categories remain visible beside the environmental result.
Respect the current guidance boundary
An assessment framed for procurement may not answer a patient-choice question. GREEN CARE was registered on 3 November 2026 and includes a 30-point checklist, but later corrections or companion material must be checked at the official record.
Practical reader check
- Point to the exact source or method sentence supporting the claim.
- Name the person, setting, date, input and decision rule that could change its meaning.
- Label the remaining uncertainty reported, inferred, contested or unknown.
Student, parent and career transfer
Make one evidence card for “State the Decision and Intended User”: source location, plain-language meaning, limit and next question. A parent or teacher can ask for one everyday explanation and one specialist explanation; both must preserve the same conditions. This habit supports healthcare sustainability, medicine, nursing, pharmacy, public health, life-cycle assessment, health economics, operations, procurement, engineering, data analysis, policy and scientific communication without promising a grade, admission, funding decision or job.
CHAPTER 2 OF 14
2. Define the Care Pathway and Population
Why this move matters
Describe patients, condition, setting, eligibility, starting point, endpoint and variations in actual care.
Did You Know? A care pathway is a sequence of events, not merely a named treatment.
Place the item on the pathway map
Describe patients, condition, setting, eligibility, starting point, endpoint and variations in actual care. Connect the wording to the patient journey, clinical choice, upstream supply chain and downstream consequence. State whether the item is an activity, material, energy flow, journey, waste stream, outcome or assumption.
Rebuild one calculation trail
Worked example: The pathway begins at referral and ends after the first stable follow-up cycle. Follow the functional unit, quantity, data source, year, geography, emission or impact factor, conversion and subtotal. Preserve enough English around every number for another team to understand what it represents.
Keep environmental comparison fair
A smaller carbon estimate does not automatically mean better care. Clinical outcomes, access, patient burden, cost, uncertainty and possible burden shifting across impact categories remain visible beside the environmental result.
Respect the current guidance boundary
A pathway average can hide high-intensity subgroups.
Make one evidence card for “Define the Care Pathway and Population”: source location, plain-language meaning, limit and next question. CHAPTER 3 OF 14
3. Specify the Comparator and Functional Unit
Why this move matters
State what is compared and the unit that makes quantities commensurable, such as one completed episode or one patient-year.
Did You Know? Changing the denominator can reverse an apparent advantage.
Place the item on the pathway map
State what is compared and the unit that makes quantities commensurable, such as one completed episode or one patient-year. Connect the wording to the patient journey, clinical choice, upstream supply chain and downstream consequence. State whether the item is an activity, material, energy flow, journey, waste stream, outcome or assumption.
Rebuild one calculation trail
Worked example: Both pathways are compared per completed 90-day follow-up episode. Follow the functional unit, quantity, data source, year, geography, emission or impact factor, conversion and subtotal. Preserve enough English around every number for another team to understand what it represents.
Keep environmental comparison fair
A smaller carbon estimate does not automatically mean better care. Clinical outcomes, access, patient burden, cost, uncertainty and possible burden shifting across impact categories remain visible beside the environmental result.
Respect the current guidance boundary
A consultation, episode and successful outcome are not interchangeable units.

CHAPTER 4 OF 14
4. Draw the System Boundary and Perspective
Why this move matters
List included and excluded stages, facilities, patient activities, supply chains, infrastructure, staff and time horizon.
Did You Know? Boundary choices are scientific and ethical choices because they decide whose burden is visible.
Place the item on the pathway map
List included and excluded stages, facilities, patient activities, supply chains, infrastructure, staff and time horizon. Connect the wording to the patient journey, clinical choice, upstream supply chain and downstream consequence. State whether the item is an activity, material, energy flow, journey, waste stream, outcome or assumption.
Rebuild one calculation trail
Worked example: The study includes patient travel, digital equipment use, clinic energy, consumables and waste but reports capital exclusions. Follow the functional unit, quantity, data source, year, geography, emission or impact factor, conversion and subtotal. Preserve enough English around every number for another team to understand what it represents.
Keep environmental comparison fair
A smaller carbon estimate does not automatically mean better care. Clinical outcomes, access, patient burden, cost, uncertainty and possible burden shifting across impact categories remain visible beside the environmental result.
Respect the current guidance boundary
No practical assessment includes everything.

CHAPTER 5 OF 14
5. Map Every Activity and Resource Flow
Why this move matters
Connect pathway steps to journeys, appointments, tests, medicines, devices, energy, water, food, accommodation and waste.
Did You Know? A readable map is the bridge between clinical workflow and environmental inventory.
Place the item on the pathway map
Connect pathway steps to journeys, appointments, tests, medicines, devices, energy, water, food, accommodation and waste. Connect the wording to the patient journey, clinical choice, upstream supply chain and downstream consequence. State whether the item is an activity, material, energy flow, journey, waste stream, outcome or assumption.
Rebuild one calculation trail
Worked example: Each remote and in-person contact is linked to its actual resources rather than a generic visit label. Follow the functional unit, quantity, data source, year, geography, emission or impact factor, conversion and subtotal. Preserve enough English around every number for another team to understand what it represents.
Keep environmental comparison fair
A smaller carbon estimate does not automatically mean better care. Clinical outcomes, access, patient burden, cost, uncertainty and possible burden shifting across impact categories remain visible beside the environmental result.
Respect the current guidance boundary
An activity map can miss informal care or unpaid work.

CHAPTER 6 OF 14
6. Report Activity Quantities and Measurement
Why this move matters
Give counts, durations, distances, masses, energy use, occupancy, source, collection method, missing values and variation.
Did You Know? Units, decimals and time periods need words that explain what was measured.
Place the item on the pathway map
Give counts, durations, distances, masses, energy use, occupancy, source, collection method, missing values and variation. Connect the wording to the patient journey, clinical choice, upstream supply chain and downstream consequence. State whether the item is an activity, material, energy flow, journey, waste stream, outcome or assumption.
Rebuild one calculation trail
Worked example: Travel distance comes from recorded origins and routing assumptions, while clinic time comes from appointment logs. Follow the functional unit, quantity, data source, year, geography, emission or impact factor, conversion and subtotal. Preserve enough English around every number for another team to understand what it represents.
Keep environmental comparison fair
A smaller carbon estimate does not automatically mean better care. Clinical outcomes, access, patient burden, cost, uncertainty and possible burden shifting across impact categories remain visible beside the environmental result.
Respect the current guidance boundary
Administrative records may not match actual behaviour. GREEN CARE was registered on 3 November 2026 and includes a 30-point checklist, but later corrections or companion material must be checked at the official record.

CHAPTER 7 OF 14
7. Identify Data Sources, Years and Geography
Why this move matters
Name datasets, suppliers, countries, facilities, publication years, access dates and relevance to the study setting.
Did You Know? Environmental factors age, and geography changes energy, transport and waste systems.
Place the item on the pathway map
Name datasets, suppliers, countries, facilities, publication years, access dates and relevance to the study setting. Connect the wording to the patient journey, clinical choice, upstream supply chain and downstream consequence. State whether the item is an activity, material, energy flow, journey, waste stream, outcome or assumption.
Rebuild one calculation trail
Worked example: Electricity factors use the study region and year rather than a convenient international average. Follow the functional unit, quantity, data source, year, geography, emission or impact factor, conversion and subtotal. Preserve enough English around every number for another team to understand what it represents.
Keep environmental comparison fair
A smaller carbon estimate does not automatically mean better care. Clinical outcomes, access, patient burden, cost, uncertainty and possible burden shifting across impact categories remain visible beside the environmental result.
Respect the current guidance boundary
Recent data can still be poorly matched to the service. GREEN CARE was registered on 3 November 2026 and includes a 30-point checklist, but later corrections or companion material must be checked at the official record.
Practical reader check
- Point to the exact source or method sentence supporting the claim.

CHAPTER 8 OF 14
8. Show Emission and Impact Factors Transparently
Why this move matters
Report factor name, unit, version, source, scope, conversions, allocation and whether upstream stages are included.
Did You Know? A factor is a modelled bridge between activity and impact, not a direct observation of harm.
Place the item on the pathway map
Report factor name, unit, version, source, scope, conversions, allocation and whether upstream stages are included. Connect the wording to the patient journey, clinical choice, upstream supply chain and downstream consequence. State whether the item is an activity, material, energy flow, journey, waste stream, outcome or assumption.
Rebuild one calculation trail
Worked example: A disposable kit is linked to a factor per kit with the database edition and boundary stated. Follow the functional unit, quantity, data source, year, geography, emission or impact factor, conversion and subtotal. Preserve enough English around every number for another team to understand what it represents.
Keep environmental comparison fair
A smaller carbon estimate does not automatically mean better care. Clinical outcomes, access, patient burden, cost, uncertainty and possible burden shifting across impact categories remain visible beside the environmental result.
Respect the current guidance boundary
Commercial databases may limit redistribution. GREEN CARE was registered on 3 November 2026 and includes a 30-point checklist, but later corrections or companion material must be checked at the official record.

CHAPTER 14 OF 14
14. Conclude With a Bounded and Updateable Comparison
Why this move matters
State what the results support, what they do not, decisive assumptions, update triggers and the next measurement needed.
Did You Know? A transparent environmental assessment is a dated decision aid, not a permanent league table.

PRACTICE LAB
Build a GREEN CARE decision trail
Student route
- Choose one original report inside this guide’s scope.
- Copy its objective and underline actor, action, context, evidence and intended decision.
- Draw the specific reasoning chain used in this article.
- Mark each step reported, inferred, contested or unknown.
- Rewrite the conclusion so it preserves the narrowest important limitation.
Parent or teacher route
Ask the learner to explain how English helps us make a care-pathway environmental assessment traceable from the clinical decision to activities, inputs, impacts and responsible comparison twice: once to a classmate and once to a method specialist. Praise a precise correction and a well-stated unknown; both show disciplined attention to evidence.
Early-career route
Create a one-page GREEN CARE audit with source link, date checked, scope, method logic, decisive wording, contrary evidence, limit and next action. A colleague should be able to continue without guessing.
Decision rule
If a shorter explanation removes the condition that made the original claim true, it is not clearer yet. Restore the condition.

FREQUENTLY ASKED QUESTIONS
Questions students and parents often ask
Is GREEN CARE final?
The official record describes a 30-point checklist registered on 3 November 2026; readers should check the live record for later updates or publication.
What is a functional unit?
It is the unit that relates resource use and impacts to the service being compared, such as one completed episode.
Does lower carbon mean better care?
Not by itself. Clinical outcomes, access, other environmental categories and equity also matter.
Why report data year and geography?
Energy, transport, products and waste systems differ across places and change over time.
Should patient travel be included?
Its inclusion depends on the declared pathway boundary and decision; the choice and rationale must be visible.
Can one environmental category represent sustainability?
No. It can answer a specific question but should not be presented as the whole social, clinical and environmental picture.

A cheerful final note
Precise English helps us make a care-pathway environmental assessment traceable from the clinical decision to activities, inputs, impacts and responsible comparison more generous, not less. People can see how their evidence and values entered the work, where disagreement remains and what a responsible next step looks like.
