WHY ENGLISH?
Turn a readability score into a transparent study of a real text for a real audience—not a verdict produced by a black box
People search for health information when they may be worried, tired or deciding what to do next. English matters because a readability score can look objective while hiding the exact document, audience, formula, calculator, exclusions and text-cleaning choices that created it.
The Readability Checklist is a reporting-guideline project for studies assessing written health information. EQUATOR registered it on 22 January 2026 to improve methodological rigour, transparency and reproducibility amid rapid growth in formulas and automated tools.
As checked on 6 November 2026, the checklist remained under development. The official record highlights incomplete method descriptions, weak justification of formula or calculator choice and insufficient detail on text preparation. CDC guidance also warns that mechanical counts of syllables and sentences are inadequate for judging communication effectiveness because audience, purpose, design and comprehension matter too.
Understanding and action
Connect scores with comprehension, accessibility and decisions.

ORIENTATION
What this guide owns
Current fact check
The official EQUATOR record identifies researchers, health-information developers, editors and peer reviewers as intended users. CDC describes its Clear Communication Index as a research-based tool with 20 scored items and says readability formulas alone do not assess most characteristics that support clarity and understanding.
Canonical boundary
This article owns reporting of studies that calculate readability of written health information. It does not own general plain-language writing, health-news reporting, accessibility conformance, literacy diagnosis or direct testing of whether a particular person understands instructions.
Worked example
Every chapter returns to a fictional study comparing the readability of public leaflets explaining when families should seek help for an adolescent fever. It is fictional, low-stakes and educational; it does not report a real school, clinical decision or guaranteed outcome.
Did You Know? For Readability Checklist, complete reporting makes appraisal possible but does not perform the appraisal, remove bias or guarantee transfer.

CHAPTER 1 OF 16
1. Identify the Exact Source Text
Why this move matters
Report title, owner, URL or archive, publication date, version, format and access date for every document.
Did You Know? Readability scores cannot be audited without the analysed text.
Reconstruct the exact text that was scored
Report title, owner, URL or archive, publication date, version, format and access date for every document. Preserve source, version, audience, language, headings, lists, tables, abbreviations and every preparation step before a formula sees the text.
Recalculate one score
Worked example: The study archives the exact fever leaflet version assessed on 6 November 2026. Use the reported formula, calculator, settings, unit counts and exclusions, then compare the result.
Keep readability separate from understanding
A formula estimates selected textual features. Test comprehension, usability, actionability, accessibility and cultural fit through appropriate additional methods.
Respect the development boundary
A preserved text may not match what users actually saw on a responsive page. The Readability Checklist is under development. This article uses its registered scope plus current official communication guidance, not invented final items.
Practical reader check
- Point to the exact source or method sentence supporting the claim.
- Name the person, setting, date, input and decision rule that could change its meaning.
- Label the remaining uncertainty reported, inferred, contested or unknown.
Student, parent and career transfer
Make one evidence card for “Identify the Exact Source Text”: source location, plain-language meaning, limit and next question. A parent or teacher can ask for one everyday explanation and one specialist explanation; both must preserve the same conditions. This habit supports health communication, medicine, nursing, pharmacy, public health, editing, information design, accessibility, research methods, journalism and education without promising a grade, admission, funding decision or job.
CHAPTER 2 OF 16
2. Define Audience, Purpose and Action
Why this move matters
State intended readers, language, health context, prior knowledge and the decision or action the material supports.
Did You Know? A short word can still be unclear when the reader lacks the concept.
Reconstruct the exact text that was scored
State intended readers, language, health context, prior knowledge and the decision or action the material supports. Preserve source, version, audience, language, headings, lists, tables, abbreviations and every preparation step before a formula sees the text.
Recalculate one score
Worked example: The leaflet is for parents deciding whether to monitor, call a clinician or seek urgent care. Use the reported formula, calculator, settings, unit counts and exclusions, then compare the result.

PRACTICE LAB
Build a Readability Checklist decision trail
Student route
- Choose one original report inside this guide’s scope.
- Copy its objective and underline actor, action, context, evidence and intended decision.
- Draw the specific reasoning chain used in this article.
- Mark each step reported, inferred, contested or unknown.
- Rewrite the conclusion so it preserves the narrowest important limitation.
Parent or teacher route
Ask the learner to explain how English helps us turn a readability score into a transparent study of a real text for a real audience—not a verdict produced by a black box twice: once to a classmate and once to a method specialist. Praise a precise correction and a well-stated unknown; both show disciplined attention to evidence.
Early-career route
Create a one-page Readability Checklist audit with source link, date checked, scope, method logic, decisive wording, contrary evidence, limit and next action. A colleague should be able to continue without guessing.
Decision rule
If a shorter explanation removes the condition that made the original claim true, it is not clearer yet. Restore the condition.

FREQUENTLY ASKED QUESTIONS
Questions students and parents often ask
Is the Readability Checklist final?
No. EQUATOR lists it as under development.
Does a low reading-grade score prove clarity?
No. It does not test audience knowledge, design, comprehension or action.
Why name the calculator?
Implementations can count sentences, words and syllables differently.
Should headings be removed?
Report and justify the rule; sensitivity analyses can show its effect.
Can one English formula score another language?
Only when there is evidence it is valid for that language and implementation.
What should accompany readability scores?
Accuracy, accessibility, comprehension, usability and actionability evidence when relevant.

A cheerful final note
Precise English helps us turn a readability score into a transparent study of a real text for a real audience—not a verdict produced by a black box more generous, not less. People can see how their evidence and values entered the work, where disagreement remains and what a responsible next step looks like.
