WHY ENGLISH?
report competency-framework development so purpose, definitions, contributors, evidence, consensus, testing, limitations and stewardship can be appraised
Competency frameworks influence education, assessment, hiring, professional development and mobility. English matters because a neat list of competencies can hide who defined good practice, which profession and career stage it covers, what evidence was used, whose voices were absent, how consensus was reached, whether the framework was tested and who will update it.
CONFERD-HP means Recommendations for Reporting Competency Framework Development in Health Professions. The published guideline contains 20 essential items across five sections: title and abstract, framework development, development process, testing, and funding or conflicts of interest. It was created through evidence synthesis, a Delphi study and workshops with an expert panel.
This guide explains the practical value of that reporting language. A competency framework can clarify expectations and support learning, but it is not automatically a curriculum, assessment, licence or guarantee of performance. Careful English keeps purpose, evidence, authority and implementation distinct so students and professionals can use a framework intelligently.

ORIENTATION
What this guide covers
Current fact check
CONFERD-HP was published online in 2022 and in the February 2023 British Journal of Surgery issue, with PubMed ID 36413510 and DOI 10.1093/bjs/znac394. The final guideline has 20 essential reporting items across five sections and was informed by a 35-member expert panel, knowledge synthesis, Delphi work and workshops.
What this guide does not cover
This article focuses on reporting the development of competency frameworks in health professions. It does not validate a particular framework, define legal scope of practice, certify individual competence, prescribe one curriculum or assessment, or assume that consensus equals empirical truth.
Worked example
Every chapter returns to a fictional framework for entry-level telehealth communication competencies for allied-health graduates. It is fictional, low-stakes and educational; it does not report a real school, clinical decision or guaranteed outcome.
Did You Know? For CONFERD-HP competency-framework development reporting, complete reporting makes appraisal possible but does not perform the appraisal, remove bias or guarantee transfer.
Warm up with eduKate’s guides to explaining scientific evidence and asking better questions in class.
CHAPTER 1 OF 15
1. Identify the Framework, Profession and Practice Level
Why this move matters
Name it as a competency framework and specify profession, specialty, career stage, jurisdiction and version in the title or summary.
Did You Know? Scope begins before the first competency statement.
Trace a framework from purpose to defensible output
Name it as a competency framework and specify profession, specialty, career stage, jurisdiction and version in the title or summary. Connect the profession, practice level, intended users and uses, definitions, evidence sources, contributors, consensus process, testing and stewardship.
Audit one competency-development pathway
Worked example: The fictional framework covers entry-level allied-health graduates using telehealth, not all clinicians at every level. Follow scoping, evidence review, stakeholder recruitment, draft domains, consensus, testing, revision, publication and update responsibility.
Keep a framework separate from a curriculum or licence
A competency framework describes expected attributes. It does not automatically prescribe teaching, prove competence, award a qualification or create legal scope of practice.
Use the published checklist accurately
A broad professional label can conceal different responsibilities. CONFERD-HP is a published 20-item reporting guideline for competency-framework development in health professions. Reporting completeness does not itself validate the framework or approve its use.
Practical reader check
- Point to the exact source or method sentence supporting the claim.
- Name the person, setting, date, input and decision rule that could change its meaning.
- Label the remaining uncertainty reported, inferred, contested or unknown.
Student, parent and career transfer
Make one evidence card for “Identify the Framework, Profession and Practice Level”: source location, plain-language meaning, limit and next question. A parent or teacher can ask for one everyday explanation and one specialist explanation; both must preserve the same conditions. This habit supports medicine, nursing, allied health, paramedicine, pharmacy, professional education, assessment, workforce planning, regulation, accreditation, human resources and policy without promising a grade, admission, funding decision or job.
CHAPTER 2 OF 15
2. State Intended Users, Uses and Decisions
Why this move matters
Explain who will use the framework and whether it supports curriculum, self-development, assessment design, recruitment, accreditation or workforce planning.
Did You Know? Purpose determines how much evidence and testing are needed.
Trace a framework from purpose to defensible output
Explain who will use the framework and whether it supports curriculum, self-development, assessment design, recruitment, accreditation or workforce planning. Connect the profession, practice level, intended users and uses, definitions, evidence sources, contributors, consensus process, testing and stewardship.
Audit one competency-development pathway
Worked example: Universities and placement supervisors use it to plan learning; regulators have not adopted it for licensing.
The same wording can have higher stakes when used for selection or progression.

This habit supports medicine, nursing, allied health, paramedicine, pharmacy, professional education, assessment, workforce planning, regulation, accreditation, human resources and policy without promising a grade, admission, funding decision or job.
CHAPTER 3 OF 15
3. Define Competence, Competency and Related Terms
Why this move matters
Provide or reference definitions for competence, competency, capability, entrustment, domain, behaviour and proficiency where used.
Did You Know? Shared definitions prevent a framework from sounding precise while remaining ambiguous.
Trace a framework from purpose to defensible output
Provide or reference definitions for competence, competency, capability, entrustment, domain, behaviour and proficiency where used. Connect the profession, practice level, intended users and uses, definitions, evidence sources, contributors, consensus process, testing and stewardship.
Audit one competency-development pathway
Worked example: The report defines a competency as an observable integration of knowledge, skills and attitudes in context.
Different professions use these terms differently. CONFERD-HP is a published 20-item reporting guideline for competency-framework development in health professions. Reporting completeness does not itself validate the framework or approve its use.
Practical reader check

CHAPTER 4 OF 15
4. Explain the Rationale and Gap
Why this move matters
Describe the practice problem, existing frameworks, policy or technology change, population need and why adaptation or a new framework is justified.
Did You Know? A rationale explains why development is worth the authority it may acquire.
Trace a framework from purpose to defensible output
Describe the practice problem, existing frameworks, policy or technology change, population need and why adaptation or a new framework is justified. Connect the profession, practice level, intended users and uses, definitions, evidence sources, contributors, consensus process, testing and stewardship.

CHAPTER 5 OF 15
5. Report Governance, Team Roles and Decision Authority
Why this move matters
Name steering, advisory and research roles, professional backgrounds, patient or public partners, who made final decisions and how disagreements or conflicts were managed.
Did You Know? Governance reveals where authority sat when wording was contested.
Trace a framework from purpose to defensible output
Name steering, advisory and research roles, professional backgrounds, patient or public partners, who made final decisions and how disagreements or conflicts were managed. Connect the profession, practice level, intended users and uses, definitions, evidence sources, contributors, consensus process, testing and stewardship.

CHAPTER 14 OF 15
14. Separate Framework From Curriculum and Assessment
Why this move matters
Explain how users may translate competencies into learning outcomes, activities and assessment while avoiding claims that the framework itself measures individuals.
Did You Know? A framework guides decisions; additional design and evidence make those decisions fair.
Trace a framework from purpose to defensible output

PRACTICE LAB
Build a CONFERD-HP competency-framework development reporting decision trail
Student route
- Choose one original report inside this guide’s scope.
- Copy its objective and underline actor, action, context, evidence and intended decision.
- Draw the specific reasoning chain used in this article.
- Mark each step reported, inferred, contested or unknown.
- Rewrite the conclusion so it preserves the narrowest important limitation.
Parent or teacher route
Ask the learner to explain how English helps us report competency-framework development so purpose, definitions, contributors, evidence, consensus, testing, limitations and stewardship can be appraised twice: once to a classmate and once to a method specialist. Praise a precise correction and a well-stated unknown; both show disciplined attention to evidence.
Early-career route
Create a one-page CONFERD-HP competency-framework development reporting audit with source link, date checked, scope, method logic, decisive wording, contrary evidence, limit and next action. A colleague should be able to continue without guessing.
Decision rule
If a shorter explanation removes the condition that made the original claim true, it is not clearer yet. Restore the condition.

FREQUENTLY ASKED QUESTIONS
Questions students and parents often ask
Is CONFERD-HP published?
Yes. The open-access guideline was published online in 2022 and in the 2023 journal issue.
How many reporting items does it contain?
It contains 20 essential items across five sections.
Does a competency framework certify an individual?
No. A framework describes expectations; fair assessment requires separate design, evidence and governance.
Is consensus the same as proof?
No. Consensus records structured agreement among participants under stated methods.
Why include patients or the public?
Competencies can affect safety, access and experience, so people receiving services may identify needs professionals overlook.
When should a framework be updated?
The report should state stewardship and triggers such as evidence, policy, technology or scope-of-practice changes.
