WHY ENGLISH?
report living systematic reviews so update rules, changing evidence, version history, author roles, current status and retirement remain understandable
A living systematic review promises to keep watching for new evidence and to incorporate it soon after it appears. English matters because that promise is made through words: what will be searched, how often, which changes trigger an update, what changed between versions and whether the review is still alive. Without precise reporting, a reader may mistake an old version for current evidence.
The PRISMA-LSR extension for living systematic reviews was published in The BMJ on 19 November 2024. It is an add-on to PRISMA 2020, not a replacement. The extension adds four new items on living-mode parameters, changes to methods, changes to results, and authors and roles for each version; it also provides tailored flow diagrams and recommendations for reporting current status and retirement.
This guide shows students, parents and future professionals why version-aware English is practical evidence infrastructure. A learner can see how conclusions change; a parent can recognise a current status statement; and a researcher, librarian or policymaker can distinguish continual surveillance from a one-off update. Clear reporting helps evidence stay useful without pretending that “latest” automatically means “best”.

ORIENTATION
What this guide covers
Current fact check
PRISMA-LSR was published in BMJ 387:e079183 on 19 November 2024, DOI 10.1136/bmj-2024-079183, PubMed ID 39562017. It is an add-on to PRISMA 2020 with four new items, new elements under existing items, tailored flow diagrams and status-reporting recommendations.
What this guide does not cover
This article focuses on reporting living systematic reviews. It does not conduct a review, judge the certainty of a body of evidence, promise real-time updating, replace PRISMA 2020 or PRISMA-P, or make every research question suitable for living mode.
Worked example
Every chapter returns to a fictional living review of school-based interventions intended to improve adolescent sleep, updated as new controlled studies appear. It is fictional, low-stakes and educational; it does not report a real school, clinical decision or guaranteed outcome.
Did You Know? For PRISMA-LSR living-review reporting, complete reporting makes appraisal possible but does not perform the appraisal, remove bias or guarantee transfer.
Warm up with eduKate’s guides to explaining scientific evidence and asking better questions in class.
CHAPTER 1 OF 15
1. Explain Why the Review Should Be Living
Why this move matters
State why new evidence is expected, why decisions need timely synthesis and why the resources are justified.
Did You Know? Living mode is a resource commitment with a public purpose.
Make the living promise operational
State why new evidence is expected, why decisions need timely synthesis and why the resources are justified. Connect scope, rationale, living parameters, continual searches, screening, synthesis, version changes, author roles, status and retirement.
Follow one review through three versions
Worked example: Rapidly developing school-sleep interventions could change recommendations within months. Track what changed in the question, methods, evidence, conclusions and public status each time.
Keep “living” separate from merely “recent”
A recently published review is not automatically living. A living review needs an ongoing process, declared update logic and visible status.
Use PRISMA-LSR as an add-on
A fashionable topic is not automatically suitable for permanent surveillance. The 2024 extension adds living-review items and elements to PRISMA 2020; it does not replace the parent statement or guarantee that continual updating is worthwhile.
Practical reader check
- Point to the exact source or method sentence supporting the claim.
- Name the person, setting, date, input and decision rule that could change its meaning.
- Label the remaining uncertainty reported, inferred, contested or unknown.
Student, parent and career transfer
Make one evidence card for “Explain Why the Review Should Be Living”: source location, plain-language meaning, limit and next question. A parent or teacher can ask for one everyday explanation and one specialist explanation; both must preserve the same conditions. This habit supports education research, medicine, public health, library science, evidence synthesis, guideline development, journalism, policy, publishing and knowledge management without promising a grade, admission, funding decision or job.
CHAPTER 2 OF 15
2. Define the Review Question and Stable Core
Why this move matters
Report population, intervention or exposure, comparator, outcomes, settings and which elements may change.
Did You Know? A stable core makes change interpretable.
Make the living promise operational
Report population, intervention or exposure, comparator, outcomes, settings and which elements may change. Connect scope, rationale, living parameters, continual searches, screening, synthesis, version changes, author roles, status and retirement.
Follow one review through three versions
Worked example: The review keeps adolescent sleep outcomes stable while permitting new digital and school-timetable interventions. Track what changed in the question, methods, evidence, conclusions and public status each time.
Keep “living” separate from merely “recent”
A recently published review is not automatically living. A living review needs an ongoing process, declared update logic and visible status.
Use PRISMA-LSR as an add-on
Unplanned scope drift can make versions answer different questions.
Make one evidence card for “Define the Review Question and Stable Core”: source location, plain-language meaning, limit and next question. This habit supports education research, medicine, public health, library science, evidence synthesis, guideline development, journalism, policy, publishing and knowledge management without promising a grade, admission, funding decision or job.
CHAPTER 3 OF 15
3. Declare Living-Mode Parameters
Why this move matters
Specify search frequency, screening cadence, incorporation timing, update triggers, pause rules and retirement criteria.
Did You Know? Parameters convert “living” from a label into operations.
Make the living promise operational
Specify search frequency, screening cadence, incorporation timing, update triggers, pause rules and retirement criteria. Connect scope, rationale, living parameters, continual searches, screening, synthesis, version changes, author roles, status and retirement.
Follow one review through three versions
Worked example: Searches run monthly, while publication occurs when new evidence changes estimates or certainty. Track what changed in the question, methods, evidence, conclusions and public status each time.
Keep “living” separate from merely “recent”
A recently published review is not automatically living. A living review needs an ongoing process, declared update logic and visible status.
Use PRISMA-LSR as an add-on
Searching often does not guarantee updating quickly. The 2024 extension adds living-review items and elements to PRISMA 2020; it does not replace the parent statement or guarantee that continual updating is worthwhile.
Practical reader check
- Point to the exact source or method sentence supporting the claim.

PRACTICE LAB
Build a PRISMA-LSR living-review reporting decision trail
Student route
- Choose one original report inside this guide’s scope.
- Copy its objective and underline actor, action, context, evidence and intended decision.
- Draw the specific reasoning chain used in this article.
- Mark each step reported, inferred, contested or unknown.
- Rewrite the conclusion so it preserves the narrowest important limitation.
Parent or teacher route
Ask the learner to explain how English helps us report living systematic reviews so update rules, changing evidence, version history, author roles, current status and retirement remain understandable twice: once to a classmate and once to a method specialist. Praise a precise correction and a well-stated unknown; both show disciplined attention to evidence.
Early-career route
Create a one-page PRISMA-LSR living-review reporting audit with source link, date checked, scope, method logic, decisive wording, contrary evidence, limit and next action. A colleague should be able to continue without guessing.
Decision rule
If a shorter explanation removes the condition that made the original claim true, it is not clearer yet. Restore the condition.

FREQUENTLY ASKED QUESTIONS
Questions students and parents often ask
What is a living systematic review?
A review with ongoing surveillance and a plan to incorporate relevant new evidence soon after it appears.
Does PRISMA-LSR replace PRISMA 2020?
No. It is an add-on used with PRISMA 2020.
How many new items does PRISMA-LSR add?
Four: living parameters, method changes, result changes, and authors and roles for each version.
Must every new study trigger publication?
No. The review should predefine how studies are incorporated and what triggers an update.
What does retirement mean?
The review leaves living mode under stated criteria; the final status and search date should remain clear.
Why does English matter here?
Readers need exact version, change, status and timing language to know which evidence they are using.

A cheerful final note
Precise English helps us report living systematic reviews so update rules, changing evidence, version history, author roles, current status and retirement remain understandable with greater generosity as well as greater precision. People can see how their evidence and values entered the work, where disagreement remains and what a responsible next step looks like.
