Everybody can be working hard and the system can still move in the wrong direction.
Enough resources may exist. Enough people may exist. Enough vehicles may exist. The failure can begin because nobody has the same picture of where the problem actually is.
A civilisation can possess the resources and still fail to coordinate them.
The one-sentence answer
Coordination fails when people, institutions or systems cannot build and update a sufficiently shared understanding of what is happening, what resources exist, who is responsible and which action should happen next.
The supplies can still be in the warehouse.
The ambulance can still exist.
The repair crew can still be ready.
The problem is connecting reality to decision.
Imagine three control rooms
A major disruption has occurred.
Control Room A believes a neighbourhood has lost water.
Control Room B believes the problem is electricity.
Control Room C believes the pumps are functioning and sends repair resources elsewhere.
Each team acts rationally from the information it has.
The combined system behaves irrationally because the pictures do not match.
This is not primarily a shortage problem.
It is a shared-reality problem.
Situational awareness is knowing enough about the present to act
Situational awareness does not mean knowing everything.
No command centre can know everything during a fast-moving event.
It means knowing enough of the important reality to make the next decision responsibly.
What has happened?
Where?
Who is affected?
Which systems still work?
Which resources are available?
What is changing?
What remains uncertain?
The last question matters because false certainty can coordinate people around the wrong picture.
A common operating picture is not one giant screen
Emergency-management systems use the idea of a common operating picture: a continuously updated overview that gives participating organisations a shared set of important incident information.

The phrase can sound technical.
The human idea is simple:
Before several groups can coordinate action, they need enough agreement about what world they are acting in.
The picture must be shared—but it also must be correctable
A shared wrong picture is worse than several incomplete pictures if nobody is allowed to challenge it.
Good coordination therefore needs a way for new evidence to update the common view.
A field team reports that the road is actually blocked.
A household reports that water pressure is still low after the dashboard says service was restored.
A hospital reports that listed bed capacity is not operational because staff are unavailable.
A warehouse reports that inventory exists but cannot be released.
The system must be able to say:
our previous picture was incomplete.
Here is the better picture.
Stale information can be more dangerous than missing information
If a dashboard says “unknown,” the operator knows to investigate.
If a dashboard confidently shows yesterday’s status, the operator may act as if the old world still exists.
This is why timestamps matter.
When was this number observed?
How quickly can the system change?
Has anyone verified the status after the disruption?
Real-time is not always necessary.
But information must be fresh enough for the decision being made.
Information has an expiry time.
A database can be accurate and still mislead
Suppose the system says ten trucks are available.
That may be factually correct.
But three have no drivers.
Two are waiting for fuel.
One cannot access the affected road.
The useful number is not “ten trucks exist.”
It is “how many trucks can perform this required journey within the necessary time?”
Coordination therefore requires data with context.
Handover is where information often leaks
One shift ends.
Another begins.
One agency finishes part of a job.
Another becomes responsible.
A patient moves from emergency care to a ward.
A repair passes from diagnosis to procurement to field work.
Every handover creates a risk that context is lost.
What has already been tried?
What remains uncertain?
Which temporary workaround is active?
Who is waiting?
The physical resource may transfer perfectly while the knowledge about it degrades.
Interoperability is partly about being able to talk
Two organisations can have excellent communications equipment and still fail to understand each other.
One uses different terminology.
One classifies urgency differently.
One database cannot read the other’s format.
One organisation assumes a code means something the other never intended.
Interoperability therefore includes technical compatibility and semantic compatibility.
The radio must connect.
The humans must also mean roughly the same thing by the words coming through it.
Too much information can also break coordination
A crisis produces messages.
Calls.
Images.
Reports.
Rumours.
Sensor alerts.
Requests.
If every signal is treated as equally important, decision-makers drown.
FEMA uses the idea of Essential Elements of Information: the information items most important for situational awareness and timely decisions.
The lesson for readers is straightforward.
Good coordination does not collect maximum information. It identifies the information the next decision actually requires.
Rumours are unverified coordination attempts
When official information is absent, slow or distrusted, people still need a picture.
They build one from friends, messages, social media, observation and inference.
Some of that information may be valuable.
Some may be wrong.
Rumours spread partly because people are trying to solve a real uncertainty problem.
A strong public-information system therefore does not merely say “ignore rumours.”
It reduces the vacuum in which rumours become operational substitutes.
Public messaging is part of coordination
If several agencies give contradictory advice, households cannot coordinate their own behaviour.
FEMA’s Joint Information System concept exists to coordinate information across agencies so public messages are less confusing during incidents.

This matters because citizens are not passive receivers during a disruption.
They decide whether to travel.
Whether to conserve water.
Whether to buy extra supplies.
Whether to evacuate.
Whether to seek medical care.
Public behaviour becomes part of the operating system.
Authority must also be clear
A shared picture does not automatically produce action.
Someone still needs authority to decide.
Who can release the stockpile?
Who can close the road?
Who can redirect crews?
Who can prioritise electricity?
Who can approve an emergency substitute?
If authority is unclear, information can circulate while nobody acts.
If authority is over-centralised, every small decision can become a bottleneck.
Coordination therefore needs both clarity and appropriate delegation.
Receiver feedback closes the picture
An upstream system says water service is restored.
The household says the tap is still dry.
Which is correct?
Possibly both.
The upstream repair may be complete while a downstream building problem remains.

Receiver feedback is not merely customer satisfaction.
It can reveal the difference between system status and human receipt.
The Lights Out Event is also an information shock
In the main Lights Out Event, external flows stop.
But so do many external information flows.
Manufacturers stop sending technical updates.
International markets no longer produce ordinary price signals.
Overseas partners do not confirm shipments.
External news, research and specialist advice may become unavailable.
People now have to coordinate from a smaller information world.
The first challenge is not merely “we have less.”
It is “we know less about what remains possible.”
Uncertainty should be displayed, not hidden
During disruption, leaders are often pressured to sound certain.
But uncertainty is part of the operating reality.
A useful picture can separate:
- confirmed: directly observed or verified;
- likely: supported by evidence but not yet confirmed;
- unknown: important information not yet available;
- outdated: previously true information that may no longer describe the present.
This reduces the temptation to convert missing information into invented certainty.
Good coordination needs dissent
If everyone is required to agree with the central picture, errors can become self-sealing.
The technician who sees a contradictory signal must be able to report it.
The nurse who sees the patient effect must be heard.
The resident who reports an unresolved local problem must not be dismissed simply because the dashboard is green.
Coordination is not obedience to one picture.
It is disciplined convergence toward a picture that remains correctable by reality.
A practical coordination test
- What decision must be made next?
- Which facts are essential for that decision?
- Which facts are confirmed, inferred, unknown or stale?
- Do the relevant organisations share the same definitions and status labels?
- Who has authority to act?
- Which information must be handed to the next team?
- What public message changes receiver behaviour?
- How can frontline evidence contradict the current picture?
- How frequently must the picture be updated?
- How will we know the decision actually improved the receiver’s situation?

- It does not claim rumours are always false.
- It does not claim coordination failures are always communication failures.
The narrower claim is that shared, timely, correctable information is an operational resource, and civilisation can waste physical capability when that resource is weak.
The civilisational lesson
Resources matter.
Infrastructure matters.
Expertise matters.
But somebody still has to know what is happening, what is available, what has already changed and who needs what next.
Civilisation is not only having the right things. It is keeping enough people oriented to the same changing reality that the right things can reach the right place in time.
