Authority must match responsibility

Accountability is not the same as blame
Blame asks who deserves punishment.
Accountability asks who must explain, correct, learn and prevent recurrence.
Sometimes misconduct deserves sanction. But a system obsessed only with blame can make people hide errors rather than surface them early.
The civilisational goal is not maximum punishment. It is reliable responsibility.
Traceability keeps complexity from becoming anonymity
In a complex system, decisions pass through many hands.
Records help answer: who observed the problem, who received the report, who decided, which evidence was used, what action was authorised and what happened afterwards?
Traceability does not require surveillance of everything. It requires enough record to reconstruct consequential decisions.
Escalation prevents local limits becoming system failure
A front-line worker may see a problem but lack authority to solve it.
A good system provides an escalation route.
The problem moves upward or sideways to someone with the required authority, expertise or resources.
Without escalation, people can know something is wrong while the system remains unable to respond.
Handoffs are responsibility danger zones
Many failures occur during transition.
A patient moves from emergency care to a ward. A project moves from design to construction. A child moves from one school level to another. A technical incident moves from detection to repair.
At each handoff, responsibility can become ambiguous.
Strong handoffs specify what is being transferred, what remains unresolved and who owns the next action.
Measurement needs an owner too

Several organisations may genuinely share responsibility for one outcome.

The receiver needs somewhere to return failure
When a service fails, the user needs a route back into the system.
A complaint channel, incident report, appeal process, help desk, regulator or responsible institution allows the receiver to signal that the promised outcome did not arrive.
A system with no return path can continue reporting success while users absorb the failure privately.
Responsibility should follow the ability to prevent harm
Not every participant has equal power.
The person with the greatest ability to prevent, detect or repair a failure may carry greater responsibility than the person with the least control.
This matters when systems are tempted to push responsibility downward onto users who cannot realistically manage the risk themselves.
Responsibility should be proportional to knowledge too
Experts and institutions often know things ordinary users cannot reasonably know.
That information asymmetry creates responsibility. A professional who recognises a serious risk may have duties that a layperson does not.
Civilisation works partly because specialised knowledge carries specialised obligations.
Automation does not remove responsibility
When software makes a decision, responsibility can become blurry.
The developer blames the data. The operator blames the software. The organisation blames automation.
But automated systems are still selected, configured, deployed and governed by human institutions.
Automation changes the responsibility map. It should not erase it.
AI makes responsibility boundaries more urgent
AI can generate recommendations, classifications and decisions at speed.
That raises practical questions: who verifies high-consequence outputs, who can override the model, who investigates error, who owns the data quality, and who is responsible to the affected receiver?
The more autonomous a system appears, the more important explicit human and institutional accountability becomes.
Responsibility must survive organisational change
People leave. departments merge. contractors change. software is replaced.
Critical obligations should not disappear during those transitions.

When something goes wrong, the system should be able to answer four questions quickly:
- Who knows?
- Who owns it?
- Who can act?
- Who checks that the action worked?
If those answers are unclear, the system contains a responsibility gap.
Singapore makes responsibility routing visible
A dense city requires large numbers of services to cross organisational boundaries: housing, transport, water, healthcare, schools, roads, utilities and public safety.
Ordinary reliability depends partly on residents and operators knowing where a problem belongs, which institution owns the response and how unresolved issues escalate.
