A referral sent is not a referral completed
A patient is discharged with a referral for community rehabilitation. The hospital sends the request, but the receiving service cannot accept it. Without follow-up, everyone may believe another team is responsible. The gold standard of pending referral tracking protects the patient across this administrative and clinical boundary.
Define the referral’s purpose and urgency
The sending team should identify the clinical question, intended service and appropriate priority. Not every referral requires the same response time, and urgency must follow clinical judgement and institutional procedures.
Record the necessary information
Accurate patient identification, relevant clinical details, contact information and required documents help the receiving team assess the request. Missing or conflicting information can cause avoidable delay.
Track acceptance, not just transmission
A digital status showing ‘sent’ is only one stage. A useful workflow distinguishes received, under review, accepted, scheduled, declined and requiring clarification, with terminology appropriate to the system.
Give unresolved work an owner
When a referral is rejected or delayed, someone must decide whether an alternative service, new information or clinical reassessment is needed. Ownership should remain clear during transfers and staff absences.
Communicate with patients
Patients should understand the expected next step, how they will be contacted and whom to approach when arrangements do not materialise. Avoid promising appointments before confirmation.
Respect privacy and consent
Information exchange must use authorised channels and appropriate safeguards. Coordination does not justify unrestricted sharing.
Measure the whole pathway
Useful measures include referral acceptance, time to appropriate service, unowned pending requests, communication gaps and patient outcomes. The number of referrals issued alone is a weak measure.
Break-test: a referral is declined after discharge
Who notices the decline, assesses the clinical consequence and arranges an appropriate alternative? The gold standard closes the loop rather than allowing the case to disappear.
