A fair decision needs more than a confident explanation
A resident disputes a fee, and two systems show different transaction states. The gold standard of dispute resolution evidence is a reliable record of what happened, which rules apply and why the final decision is justified.
Define the disputed issue
Separate factual disagreement, policy interpretation and requests for discretionary review.
Collect relevant records
Use appropriate transaction identifiers, timestamps, communications and authorised service records.
Assess source reliability
A screenshot, provider record and ledger entry may represent different stages. Interpret each in context.
Preserve procedural fairness
Give relevant parties an appropriate opportunity to clarify information under applicable procedures.
Document the reasoning
Explain how evidence supports the decision and where uncertainty remains.
Verify corrective action
If records or payments require correction, confirm that the authorised outcome is implemented.
Break-test: two authoritative systems disagree
Can the agency trace the sequence and resolve the discrepancy without assuming one system must always be correct?
