Revision error recurrence is the return of a previously corrected failure because the repair changed the visible answer without sufficiently changing the knowledge, selection or execution mechanism that produced the error.

A correction can solve one question without repairing the mechanism that produced the error. The learner may copy the right step, remember the teacher’s wording, or succeed while the correction remains visible. Recurrence appears later when the cue changes, time passes, a competing method appears, or exam pressure returns. Durable repair therefore requires identifying the failure class, rebuilding the missing distinction or route, removing support and retesting after delay and under a changed surface. The real receipt is not “got it right immediately after correction.” It is “did not reproduce the same mechanism later.”

“Careless” may actually mean:

- confused concepts;
- wrong method selection;
- unit/representation mismatch;
- retrieval latency under pressure;
- checking routine missing.
If the cause label is wrong, the repair will be wrong too.
