A failed simulation should lead to safer performance
A clinician misses a simulated alarm escalation step. The gold standard is not merely repeating the same assessment until a pass is recorded; it is diagnosing the skill gap, providing targeted practice and verifying competence.
Identify the specific gap
Was the issue alarm recognition, clinical interpretation, device operation, communication or an unclear workflow? Different causes need different responses.
Check the scenario itself
An unrealistic or outdated simulation may contribute to poor performance. Validate the exercise before attributing every problem to the learner.
Provide focused instruction
Use approved procedures, suitable demonstrations and supervised practice matched to the role.
Support deliberate practice
Rehearse the difficult decision with appropriate variation rather than memorising one scripted sequence.
Reassess independently
A new but relevant scenario can test whether competence transfers beyond the original exercise.
Document appropriate outcomes
Record required remediation and reassessment according to institutional requirements while protecting professional dignity.
Break-test: the participant passes only with prompts
Can the programme distinguish assisted performance from independent competence before authorising relevant work?
