A drill is useful only when it changes readiness
A clinical team completes a rare-event device simulation. Everyone participates, but the backup escalation route is missed. Performance review identifies what happened, why and which corrective action is needed.
Define the intended competence
Specify device operation, safety decisions, communication and escalation tasks relevant to the scenario.
Review observable evidence
Use simulation records and facilitator observations rather than relying solely on confidence or impressions.
Separate skill and system factors
Outdated instructions, unclear roles and unrealistic scenario design may contribute to poor performance.
Provide constructive feedback
Identify specific actions and reasoning that need improvement.
Assign follow-up
Training, workflow changes or reassessment should have clear responsibility.
Verify improvement
A later scenario can test whether the relevant capability has improved.
Break-test: the team performs well only when warned about the fault
Can the assessment evaluate recognition under realistic uncertainty?
